WELCOME TO THE HEALTH OFFICE!Nanette Pasion, the Health Aide, provides first aid and emergency care to students and maintains cumulative health and immunization records for all students. In addition, the health aide can help students obtain health related resources available in the community. Office hours are 8:15am-4:30pm Phone: x40060 The district nurse, Susan Houser, is available on site every Thursday, 7:45am-3:15pm. She can assist in identifying and following up on any and all students who may have chronic, acute, or latent health problems. Her officephone is 650-367-9750 x45332. Please note that the health office can not dispense any over the counter medication to students without a doctor's order and parent/guardian's permission on file in the health office. New Immunization Requirements as of July 2019
Health Forms
- Medication Form (English) - This form is available for students that need to take any over the counter or prescribe medication, during school hours.
- Medication Form - Spanish
- Diabetes Management Form - Diabetes management at school for Type I and Type II diabetic students.
- Seizure Action Plan - School Action Plan form for students with Seizure Disorder
- Allergy Action Plan/Anaphylaxis Form - to be on file should the student have a severe food, latex or insect allergy.
- Asthma Action Plan - to be used for asthma management in school.
